  1. BOOST Collaborative Learning Session 2 Storyboard Template Instructions • Use this template as your guide. Have fun and be creative. Your storyboard will be posted at LS2 for other teams to view. • For your 5-minute Storyboard Presentation: Fill in the template as you like and email to lbeamish@cfenet.ubc.ca by Monday, March 12th • Arrange any extra materials that compliment your storyboard (e.g., photos, forms, tools, etc.).

  2. Your Collaborative Team • Tell us who you are • Team name, team members and roles • Brief description of your practice/program, your physical space, and the population you serve

  3. Aim Statement • What are you trying to accomplish? • What aspect of care are you trying to improve? • Why is this important to do now?

  4. Describe your Population of Focus • Who are your clients?

  5. Changes Tested • What small tests of change have you tried? • What were you measuring? • Did you implement them?

  6. Lessons Learned • Share your progress so far- what have you learned about your POF, partnerships, etc. • Share any lessons learned or opportunities for improvement you encountered • How did you address these?

  7. Looking forward… • What is next?

  8. Contact Information • Team contact information
