This document outlines the rationale for setting Donor Management Goals (DMGs) as part of a quality improvement initiative in organ donation. Led by Cindy Jacobs, RN, BSN, CPTC, the project aims to enhance donor criteria, including Standard Criteria Donors (SCD) and Expanded Criteria Donors (ECD), while ensuring patient care aligns with donation practices. Key metrics such as MAP, CVP, sodium levels, and pH are crucial for optimizing donor management. By emphasizing aggressive care for patients with catastrophic brain injury, we can improve outcomes for both donors and transplanted organs.

“Rationale for establishing donor management goals.” Cindy Jacobs, RN, BSN, CPTC Professional Development Director
Donor Management Goals (DMGs) Standard criteria donor (SCD) OTPD = 4.30 Expanded criteria donor (ECD) OTPD = 2.50 Donation after cardiac death (DCD)OTPD = 2.75 Overall OTPD = 3.75
DMGs = final labs prior to surgical recovery • UNOS Region 10 Donor Management Goals Quality Improvement Project: • MAP > 60 mmHg • Pressors < 1(one pressor plus Vasopressin to treat D.I. meets goal) • CVP < 10 (or normal serum osmolality 285-295) • Sodium < 155 • pH 7.25-7.5 • pO2 > 300 on 100% O2 (or equivalent , P/F ratio > 3)
Maximizing every gift. Care that is good for the patient is good for the donor. Patients with catastrophic brain injury receive aggressive patient care. Transplanted organs also benefit from aggressive patient care.
