  3. 2013 Community Services Guide-at-a-Glance Overview • Continuation of Workgroup’s activities from last year • Resource Directory of local area agencies and organizations serving Prince George’s County, Maryland residents • For over 13 years, has been used by local government agencies, community organizations, and human service providers to link clients to available services. • Newly-revised and updated booklet from original 2000 Guide

  4. 2013 Guide-At-A-Glance Revisions • Includes Spanish translation of topics and service descriptions. • Provides web site addresses for nearly all listings • New tabbed alphabetical index that enables users to quickly locate information ~ Serves as a time-saving feature that will enhance customer service capabilities • Guide is available as both a PDF and a printed booklet ~Allows unlimited download access to organizations and residents who do not receive hard copies

  5. 2013 Guide-At-a-Glance Distribution Priorities *Predetermined copies were reserved for organizations based on prioritized lists Database was created that prioritized first and must haves of booklets *Revised letter was sent, via email, to organizations to prompt for pickup.

  11. 2013 Guide-At-A-Glance Follow-up • For claimed booklets; • Follow-up calls • Call organization to verify pick-up and that distribution occurred from their site • For unclaimed booklets • Currently in process of creating a different method of distribution • Pickup of Guides At PGHD HQ after meeting

  12. Ultimate Goal • Community Services Guide-At-A-Glance will quickly become one of the most valuable tools used by County organizations for serving clients and the public, while ultimately helping to improve the health and quality of life of all Prince George’s County residents.

  13. Potential Sources of Funding • National Organizations Support for State Oral Health Programs • ACA SHIP and the Aging and Disability Resource Center (ADRC) Options Counseling for Medicare Medicaid Individuals in States with Approved Financial Alignment Model

National Organizations Support for State Oral Health Programs • Funded by the Center for Disease Control and Prevention • Will support states efforts to perform essential public health functions related to oral health, address oral diseases, and support coordination of oral health efforts with state chronic disease programs and other partner organizations • Threshold Amount: $1,750,000

ACA SHIP and AGING & DISABILITY RESOURCE CENTER (adrc) Options Counseling for Medicare Medicaid Individuals in States with Approved Financial Alignment Model • Funded by the Centers for Medicare and Medicaid Services • Provide options counseling to Medicare-Medicaid individuals to ensure that these individuals have access to an unbiased and consumer friendly source of information counseling to help them make informed decisions about options • Award Ceiling; $1,000,000

Chronic Diseases resource guide • Draft Resources Guide developed late in 2012 • Several challenges evolved that put the project on hold until recently • Approximately $6,000.00 still available from the Maryland Community Health Resources Commission to support development of the Resources Guide • Minor editing to include USDA’s MYPLATE.GOV icon and website link.

  20. CHRONIC DISEASES RESOURCE GUIDE • For now, Resource Guide will be web-based until we find additional funding to print hard copies if desired (14 plus pages long) • Thumbnail on Health Department home page for free download by the public • MYPLATE.GOV magnet also being developed.
