  3. Forms— • Must be completed within 30 days of eligibility. (no later than July 30, 2013 for a July 1 effective date) • If not done in a timely manner, next period of enrollment will be during Open Enrollment. • November 1 – 30, 2013 for an effective date of January 1, 2014. --------------“Open Enrollment”

  4. Enrollment • Participation: The health plan is not mandatory. The basic life insurance is paid by the salary-paying unit. Supplemental life is paid by the participant. • Incapacity Leave: You must be enrolled prior to the granting of Incapacity Leave in order to have health and/or life insurance benefits under the Conference plans during your Incapacity Leave. • Retirement: The BOP’s funding of retirement health and life insurance premiums will be based on the number of months you participated in the insurance plan prior to retirement. You must be enrolled for at least the last 5 consecutive years immediately preceding the date of retirement to have access to the plans after retirement. Details are located in the BOP’s report of the Conference Journal.

  5. Life Insurance • If enrolled in the Conference Life Insurance plan: • $25,000 term life coverage on the active participant. • $10,000 term life coverage on eligible dependents (if family coverage is elected). • Effective July 1, 2013.

  6. Beneficiary • Must name a primary beneficiary. • May name a secondary (or contingent) beneficiary. • Must sign and date life insurance enrollment form.

  7. Life Insurance Rates • $18.75 monthly – Family coverage • $14.50 monthly – Single coverage • Non-contributory Plan –Salary-paying unit must pay this benefit on behalf of the participant.

  8. Supplemental Life • Participant may purchase supplemental life insurance at their own expense • Age-banded rates • Minimum purchase $25,000 • Maximum purchase $200,000 • May require a Statement of Health and approval by MetLife, dependent upon amount requested

  9. Health Insurance • Self-funded • What is self-funded? • ASO—Administrative Services Only • What does this mean? • Conference pays claims and administration

  10. Health Rates Church Portion: • Church pays a flat rate of $918.02 per month for each participant enrolled in the health plan. Personal Portion: • Individual pays a portion based on the level of coverage elected. • Personal portion is to be with held from salary on a pre-tax basis. Salary paying unit should submit the entire amount on a church check.

Specialty Drugs are filled by CuraScript. 30-Day supply = one co-pay

  14. Dental • Preventative Care 100% (UCR) • Cleanings twice a year • Basic Restorative 80% • after deductible • Major Restorative 50% • after deductible • $50 Annual Deductible • $1500 annual maximum dental benefits • $1000 lifetime maximum for orthodontics • Dependents under the age of 19

Flexible Spending Accounts(Optional for those enrolled in health plan) • Annual Election—Section 125 (IRS code) • If you do not use the funds, you lose the funds. • Must re-establish these accounts each year during Open Enrollment. • Medical Reimbursement Account • Funds are withheld from your salary on a pre-tax basis to cover un-reimbursed medical, dental, vision. • Dependent Day Care Account • Funds are withheld from your salary on a pre-tax basis to cover the cost of dependent day care. • www.probenefits.com

Insurance changes should be reported to Caroline Thornton, Benefits Manager. • Example: • Birth of new baby • You must notify our office within 30 days of the birth • Need name, date of birth • Coverage effective the date of birth • Next opportunity—Open Enrollment
