  7. The Next Six Months • Continue and build practice engagement. Encourage nonparticipating physicians and NPs to join your work. • Registry: If you have one, figure out how to incorporate its use into practice flow. If you don’t, continue to work within your organization or practice to get this capability. Use billing inquiries to at least know your denominator and who you are missing. • Sustain achieved improvement. Update flow diagram as needed. Analyze failures. Monitor processes.

  8. The Next Six Months • SMS: Incorporate patient and family goals into management strategies. Consider motivational interviewing. Engage community organizations and resources. • Environmental controls • Medical/Legal Partnerships

Questions • How do you envision the spread of quality measures in your local area? • How have you engaged your senior leadership and how receptive have they been? • What can we do better as we reach out to the practices? • What is the best way to communicate- in person learning sessions, conference calls, listerv? • What resources do you need at the practice level for QI? • What other QI projects have you been working on or thinking about initiating in your area?
