  28. Proactive Patient Management PPM provides a quick look at MHS Population Health Portal data. This includes Action Lists use to track patients with chronic illnesses and those who are due/overdue for clinical preventative services. PPM provides the ability to auto-generate patient contact letters and Diabetic Report Cards that can be mailed fresh from the printer. PPM also auto-generates Population Health reports as well as CPG’s and HEDIS metrics reports. Each patient name displayed is linked from the Pap, Mammo, and Colon Screen Overdue Lists to the ‘Patient Summary’ view

  35. PCM Patient Referrals • PCM Patient Referrals displays a list of all the referrals made for your enrolled patients. From this list Users can drill down to the details of each individual referral performed for a selected patient. The ‘Referral’ details can be exported to .pdf format for the purpose of printing. • Access to the PCM Patient Referrals requires either a Signature Class >2 or one of the following Stand-Alone User Role security keys in CHCS: KWMS1 as well as a CHCS Provider ID.
