How are forms used on your website? • Why are you collecting information? • What information are you collecting? • Is it PII (Personally Identifiable Information)?https://www.gsa.gov/reference/gsa-privacy-program/rules-and-policies-protecting-pii-privacy-act • PII info must be handled securely through UIT • If using third-party, vendor protocols must be reviewed and approved by UIT

  8. How are forms put on your website? • Handwritten code? • Third-party form wizard? • Linking to an external form? • Asking UIT or Web Marketing for help?
