Tourist Passport Application Overview • This presentation will explain the following steps in applying for a Tourist Passport: • Complete Form DS-11: Application for a U.S. Passport • Get passport photo taken • Obtain evidence of U.S. Citizenship • Obtain a copy of your ID • Submit all of the above as a package to an application facility • Pay the passport fee • This brief references the Department of State website located here: US State Department Bureau of Consular Affairs

  3. Completing the DS-11 • The Form DS-11 can be filled out online or completed by hand • Go to https://pptform.state.gov/ to begin filling the DS-11 out online or skip to step 3 to start filling • Select that you have read the Privacy and Computer Fraud notices, then click Submit.

  5. Completing the DS-11 • Enter the required information and click Next Enter USN Enter Military Member

  6. Completing the DS-11 • Enter the required information and click Next Enter the address you want your passport sent to. Enter your _____ @usna.edu email address Enter a cell phone number.

  7. Completing the DS-11 • Travel Plan information is not required. If you don’t have upcoming travel plans, click Next.

  8. Completing the DS-11 • Enter emergency contact information. This should be a parent or legal guardian.

  9. Completing the DS-11 • If you don’t have a tourist passport book or card, select NONE.

  10. Completing the DS-11 • Enter the required information and click Next

  11. Completing the DS-11 • If you are known by other names, enter the information. Otherwise, click Next.

  12. Completing the DS-11 • Review all of your information for accuracy. Click Next.

  13. Completing the DS-11 • You are applying for a Passport Book. Select processing and delivery as required, then click Next.

  14. Completing the DS-11 • Print your completed DS-11. Select locate to find an Acceptance Facility near you. If on the Yard, the nearest facility is the Post Office in 7th Wing. When you have reviewed this page, select the acknowledge box and click Create Form. Print the form for submission. After printing the DS-11 form, DO NOT SIGN it until told to do so by an authorized agent

Passport Photo • The USNA Photo Lab is open on Thursdays from 0800-1500 for walk-ins. • MIDN can also have passport photos taken at some application processing agencies, as well as local CVS and Kinko’s stores • When submitting your passport photo, ensure that the photo is not attached to the DS-11 form

Obtain Evidence of U.S. Citizenship • One of the following must be submitted as primary evidence of U.S. Citizenship: • Previously issued, undamaged, and fully valid U.S. Passport (5 year for minors or 10 years for adults) • Certified U.S. birth certificate (must meet all of the following requirements):  • Issued by the City, County, or State of birth • Lists bearer's full name, date of birth, and place of birth • Lists parent(s) full names • Has date filed with registrar's office (must be within one year of birth) • Has registrar's signature • Has embossed, impressed, or multicolored seal of registrar • Consular Report of Birth Abroad or Certification of Birth • Naturalization Certificate • Certificate of Citizenship

Obtain Evidence of U.S. Citizenship • If you do not have primary evidence of U.S. citizenship or your U.S. birth certificate does not meet the requirements, you will need to provide Secondary Evidence of U.S. Citizenship. • Secondary Evidence requirements are discussed here: US State Department - Citizenship Evidence • Provide primary evidence if possible, secondary evidence makes the process much more difficult NOTE: All citizenship documentation must be a certified original copy, not a notarized copy or photocopy.

  18. Obtain Proof of Identity • Make a photcopy of the front and back of your miltiary ID • Must be on plain white, 8.5 x 11” standard paper stock • Photocopy must contain images on only one-side of each page submitted • You will also need to present your military ID in person at the time of application • In the event your military ID is unavailable, other accepted forms of ID can be found under the following link: US State Department - Identification

  19. Submit the Application package • Submit your completed DS-11, evidence of U.S. Citizenship, Passport photo, and photocopy of your ID as a single package to any nearby application acceptance facility. Annapolis facilities are shown below. • Total cost for a new Passport book is $135. Routine processing time is 4-5 weeks.

  20. Passport Receipt • When you have received your Tourist Passport, enter your passport information into MIDS and let your company training officer know that you’ve completed the process.
