  3. CHALLENGES & BARRIERS • Foster personal ownership, versus just charity • Everyone pays (something) -Include creative “payment” options -( e.g. education, volunteerism, etc.) • Dignity and Self-esteem • Confidentiality • Equal medical treatment for all, essential for lifestyle change

  4. A NEW PARADIGM • Focus on prevention, versus just treatment Education Wellness (Know your numbers) • High quality treatment • Convenient access - Location - Hours of operation • Affordable

Asset-Based Community Development • Invent ways to close the gap between ideal vision and current reality ( Exercise vs. Activities for Life) • Identify potential means • Shared visioning • Constantly build and rebuild relationships • Strengths and capacities of the parties involved • Investment, creativity, hope and control

  6. CHW MISSION1998-Present To Provide “Good enough for me” family healthcare for the community through improved access, A heightened and unrelenting focus on education and prevention and state-of-art treatment and control

CHW-A New Paradigm • Focus on prevention, vs. just treatment • Foster personal ownership ( Contract to Care) • Dignity and self-esteem • Convenient access • High quality treatment • Affordable • Equal medical treatment, regardless of ability to pay • Community center for health and wellness education, outreach • Outreach workers-(community members as connectors.

Wichita Black Nurses Assoc.1973-2008 - 35 years of service • Define & determine nursing care for consumers for optimum quality of care by acting as their advocates • Health screening in the community • Prevention and Wellness programs • Act as a change agent in restructuring existing institution and/or help to establish institutions to suit our needs • The WBNA- One of the main forces behind the CHW.
