Open Enrollment • New Medical Provider – United Health Care • Will continue to have both a Base and a Buy-up Plan • Flexible Spending Account available with the Buy-up Plan • Health Savings Account available with the Base Plan • Dental and Vision Plan remains with Aetna • New year begins October 1, 2020

  4. United health care plans UHC Base plan UHC Buy-up plan Buy-up Plan deductible is $3,000 Office visits are $35 for Primary Care and $70 for Specialist. Emergency Room visits will now cost the deductible plus 20%. • Base Plan deductible increased to $6,250; up from $5,000. • Office visits after deductible has been paid are $35 for Primary Care and $70 for Specialist. • Emergency Room visits are the deductible plus $300.

HSA (Health Savings Account) • This feature continues for Base Plan participants and will renew in December 2020 and will begin January 1, 2021. • ACI currently contributes $25 per month for employees who also contribute the same or greater amount. • This feature can be used for any health care related expense, as long as the funds are in your account. If you have money remaining and you go to Medicare, you can use these funds to assist with Medicare as well. • FSA (Flexible Spending Account) • This program feature is available for those who elect the Buy-up Plan. • It’s a way for employees to save money on a pre-tax basis to pay for any related health service. • Health services include Medical, Dental, and Vision. • This is a use-it-or-lose-it program, so employees need to calculate their needs closely so they don’t lose anything at the end of the year.   8. Open Enrollment things to do • Review all handouts and information so you can make an informed decision • Complete the enrollment forms with all dependent information • Submit all forms to Karen Miller no later than 9/28/2020
