What IS Expected of You • Use the provided marketing materials to get the word out about CoOportunity Health • Use the talking points to ask the members a few questions • Provide members a brochure and log their referral information into the GBL portal or advise them to call the toll free number, 1-855-274-2383

The Referral Process Step by Step Guide • Step 1: Ask one of the following questions to the member as you’re helping them: • Have you heard about the new health law and how you can get affordable health insurance now? • Do you currently pay for your own health insurance? • Have you heard about our new health insurance program with CoOportunity Health? • Do you have health insurance through your employer? • Step 2:Respond using the talking points and provide them with a brochure • Step 3: Refer the member to GBL using the online portal or toll-free number • Enter the members’ information using the “Make a Referral” tab on IowaCreditUnions.com/CoOportunity (best option) • Direct them to the toll-free number: 1-855-274-2383 • Call GBL from the credit union and have them set up an appointment

You may qualify for tax subsidies that will make your premiums less expensive. Simply call the toll-free number or let me complete a referral using our online portal. • If you have health insurance through your employer then an individual plan from CoOportunity Health likely isn’t the best solution for you. You should talk to your employer about your options and make sure your dependents are covered under your plan.
