  2. Rectumtumoren • Zelfde histologie (adenocarcinoma 98%) als colontumoren maar 2 belangrijke verschillen: - relatie tov anale sfinctercomplex (sfinctersparende chirurgie?) - gefixeerd orgaan in kleine bekken (RT!) • Lokaal recidief na chirurgische resectie frequente presentatie

  3. Nieuwe behandelingen • TME (chirurgie): total mesorectal excision • In advanced disease: neo-adjuvante therapie (radiotherapie en chemotherapie) MRI belangrijke rol!

  4. Normale anatomie en stagingTNM • Vandaag T staging • N staging : vandaag nog altijd gebaseerd op grootte van de klieren • M staging : CT abdomen

  5. Normale anatomie en staging • Based on TNM criteria: • T staging • T0 no invasion • T1 invades submucosa • T2 invades muscularis propria • T3 through muscularis propria • T4 into adjacent organs (inc. peritoneum)

  6. Normale anatomie en staging MRI geen onderscheid tussen T0/T1, hiervoor is endo-echo wel goed

  9. Mesorectum on MRI → TME = en bloc resectie met mesorectale fascia als begrenzing → Door deze behandeling: local recurrence ↓

  13. T1 MRI geen onderscheid tussen T0 (mucosaal) en T1 endo-echo wel dit onderscheid

  19. Normale anatomie en staging • T0 en T1: local resection • T2 en early T3: TME • Advanced T3: RT/CT + (extensive) surgery • Disseminated: palliation (stent)

  20. Circumferentiele resectie marge(CRM)

  21. Good T3 = grote CRM *Laag risico op lokaal recidief *No or minimal adjuvante R/

  22. Bad T3 = kleine CRM *Hoog risico lokaal recidief *Extensive adjuvante R/

  23. Besluit • T staging maar zeker even belangrijk CRM (good T3 and bad T3) • MRI zeer goed voor beoordeling mesorectale fascia en dus pre-therapie planning • Relatie van tumor tov sfincter en dus pre-therapieplanning
