  4. NOVA RHIO: MEDS-ED PILOT PROJECT AT INOVA ALEXANDRIA HOSPITAL

Why MEDS-ED? • The MEDS-ED Link Pilot Project – supported by Inova, GE, Picis, Audacious Inquiry, GMU and the Commonwealth of Virginia – has the goal of providing electronic access to patient medical histories in hospital EDs and integrating with hospital ED systems. • NoVaRHIO started with MEDS-ED as the benefit of health information in emergencies is recognized by most – MEDS-ED contributes to gaining public support. • Assists physicians, hospitals and patients - in treatment, in quality of care and in medication reconciliation. • Provides foundation for a full HIE and integration with ED systems as a result of contribution of partners and development of CCD platform. • Dovetails with the NoVaRHIO “File-for-Life”, a basic paper-based personal health record summary. initiative has a potential health benefit and builds awareness of the value of health information. 5

NoVaRHIO’s Menu of Services and MEDS-ED Future Path • Extend MEDS-ED to labs, radiology, physician community desktop, immunizations, discharge summaries • Build upon “File-for-Life” effort with PHR+ tailored to specific healthcare groups and providing / enabling surround services; enhanced core of personal health information at time of emergency medical system (EMS) care • Connect to COV-HIE • Investigate possible connections with VA / DoD • Support regional provider efforts on Stage 2 Meaningful Use • Provide value as a neutral organization in rapidly changing technology and data availability environment; adaptation of leading edge and proven mobile HIE type technologies and applications from other U.S. regions. 9

Disruptive Influences and Challenges for HIE Development • Concurrent hospital and health systems development and promotion of shared electronic health record with internal capability of sharing information and messaging among hospital and attending physicians. EPIC electronic health record Uncertain physician acceptance • Cloud computing reduces investment expense for an HIE, but pricing of services and products is controlled by the cloud vendor, with the HIE receiving a percentage of the gross income.

Disruptive influences and Challenges for HIE Development • Sustainability of funding models for HIE in Northern Virginia Grant dependence not a sustainable model (CareSpark) Business model for mix of sponsorship fees and subscription and transaction fees for users not yet established • Rapid technology change and associated technology risk • Uncertain regulatory environment • Dynamic competitive and market environment – market participants are suppliers, partners and competitors at same time.
