  3. To log into MyHealth portal you need to: • Visit MyLMU.edu • Log in with your your LMU username and password • From the top Menu, choose System Logins • Select MyHealth • All forms are listed on the home page

  5. Consent for Treatment form • Review and acknowledge this form by checking the box and submit as final • Click on the Consent for Treatment on the homepage

  6. Students under 18 years old: • Download the Consent for Treatment of a Minor located in DownLoad Forms.

  7. Print this form and have it signed by your parent or legal guardian and submit to SHS.

  8. Notice of Privacy Practice Review this form and acknowledge by checking the box and submit final Click on the Notice of Privacy Practices form on the homepage

  10. Students who were bornin a country with high incidence of TB (see list on the right) MUST be tested for TB but do not need to complete the TB Questionnaire. Students who were not born in a country with high risk TB, MUST complete the TB Questionnaire and will need to be tested for TB only if the answer to any item on the TB Questionnaire is "yes'.   11. Answer the 6 questions on the questionnaire and click Submit Final • To answer, Click on the TB Screening Questionnaire on the homepage.

To be compliant with the MMR requirements you MUST complete ONEof the 3 options on the portal • Option one: enter the dates of your MMR immunizations • Option two: if your Measles, Mumps and Rubella immunizations were administered individually, enter the dates on item 2,3 and 4 • Option three: if you had a blood test to show immunity, enter the results on item 5,6 and 7

Students born in a country with high incidence of TB or students who answered “YES” to any of the questions outlined on the TB Risk Screening QuestionnaireMUSTcomplete ONE of the following options within the last 12 month • Option one: enter the date of your skin TB test. • Option two: enter the date of your Quantiferon TB blood test. • Option three: enter the date of your T-spot TB blood test.

  15. If you had a positive skin or blood test you must submit the date of a chest x-ray report

To upload your records: • Click on the “Add immunization record” button at the end of the Immunization Entry Form and upload the copies of your required records, such as immunization records, and if applicable TB test results, x-ray report and Consent to Treat a Minor form. • Submit Final to complete the process.

To verify if you are compliant with the SHS Entrance requirements: • Allow 10-14 days for SHS to process your forms. • On MyHealth portal, go to the immunization section • Verify the status of your immunization /TB requirements. • The Status color will be green and will say “Compliant with Standard Requirements” once requirements are completed • If the Status is red, you have an incomplete requirement, therefore you are NOT compliant.
