  3. Landmarks’ Network • Landmark is available for groups up to 199 employees • Members have access to Landmark’s quality network of 1500 practitioners. • Members can also submit providers to be added to Landmark’s network.

Major Product Categories • Standard ProductsCoverage for chiropractic or acupuncture services to treat acute conditions resulting from injury or illness. • Expanded ProductsIn addition to coverage to treating acute conditions, the expanded plan covers services for preventative, maintenance, and wellness care for any condition and/or injury or illness.

  5. Small Group vs Large Group • Small Group: 2-50 employees • 20 office visits a year • Choice of $15 or $20 copay • Choice of Chiro Only or  Chiro+Acu • Large Group: 51-199 • 20 or 30 office visits a year • Choice of $10, $15, or $20 • Choice of Chiro Only, Acu Only, or Chiro+Acu

  6. Chiropractor Services include: • Exams • Chiropractic Manipulation • Conjunctive Physiotherapy • And X-rays

  7. Acupuncture Services include: • Acupuncture • Electro-acupuncture • Moxibustion, herbal healing therapy • Acupressure, only when acupuncture is contraindicated
