Description • What is the London Inequalities Collaborative? • The London Inequalities Collaborative is a time limited exercise to test the effectiveness of applying the collaborative methodology developed by the Modernisation Agency, to reducing health inequalities. The first topic is Stop Smoking Services. Inequalities Collaborative Ealing and Hounslow

Description • What is my job? • Community Health Development Project Manager (Inequalities Collaborative for Smoking) • The project manager will have the responsibility for addressing the health Inequalities agenda. The main focus of the post will be to increase the number of people who stop smoking by working in partnerships with a wide range of local organizations, community groups and the Stop Smoking Service. AIMS • 1)To implement quality improvement techniques in the Stop Smoking servicesand other services • 2)To increase community engagement from hard to reach groups • 3)To increase the quit rate from hard to reach groups • 4)To increase accessibility of the stop smoking services from hard to reach groups Inequalities Collaborative Ealing and Hounslow

The Somali community are hard to reach; the Somali men mainly use the Somali cafés, by addressing them directly, with a focus on smoking cessation and raising their awareness of the accessibility of the service will increase. It will reduce the health inequalities, allow groups of the community (Somali men) to access the service, increase the level of involvement of the general community with information and awareness of the health implication associated with smoking Inequalities Collaborative Ealing and Hounslow

PDSA sample (the Green cafés) • DO • We had mainly provided materials from the stop smoking service coupled with translated material (in Somali) and C, O monitor, • As we communicated with the crowds of young men who where keen to test the C, O in their lungs, we had times when it was either very quite or very crowded. • We had manage to raise awareness and let them ask us questions about the service,We where in the spot for a few hours on Saturday, Inequalities Collaborative Ealing and Hounslow

  19. PDSA sample (the Green cafés) Study We had managed to get more people then expected, there where nearly 70 people who where monitored using the C,O and over a hundred leaflets handed out, English and translated, More……….. Inequalities Collaborative Ealing and Hounslow

PDSA sample (the Green cafés) Act We had decided to do it again because it was a good way of engaging with the community, but also we are going to set up the table differently this time in order to highlight on the stop smoking service information, we will tell them about the problem of smoking and the service and then monitor them or the other way round to deliver the massage to all of them More….. PDSA’s That had been achieved and are in process • Developed a links between community organisers and the SCS. • Trained more people from the community as CAs. • Increased awareness sessions about the service as well as the health risks related to smoking. • Local Businesses and organisations are getting involved in local activities promoting better health. • Contacting the media • Providing translated material • Increased network. • And More…..
