P1 - Hounslow Health Campaign Walk and Cycle Exercise Referral Programme
Background • Campaign designed for Active Car Owners with influence on Status Seekers – like walking and see the benefit of walking and cycling but don’t do much at the moment • Reducing car use should make them feel good • Positive messages around health / environment • Ease of travelling around the area and discovering new places / meeting new people
Campaign • Walk and cycle exercise referral programme: • Standard package (available to all residents) • Enhanced package (available to patients who meet defined criteria – access to a bike) • New campaign but utilises existing resource – www.hounslowtravelactive.org to register patients, create patient profiles and to survey and monitor progress
How it works • Patients register to Hounslow Travel Active website (standard package) or return one of our postcards (enhanced package) • They are then contacted by walk training company or cycle training company to plan their programme (4 or 6 weeks) • Walk or cycle trainer will travel to patients house to ensure participation and help patients who need the most support
Programme • Set goals on website and upload their walk and cycle data to meet their goal • Given an active travel pack plus pedometer or cycle computer • Invited to led-rides and group walks during the year • Follow up phone call after programme and then further calls at 3 and 6 months to give further support and motivation
Campaign • Targeted to 10 surgeries in Hounslow to control demand • Official launch during Bike Week (16-24 June) but GP’s have started to refer patients • Run until March 2013 with potential to extend • Can report on weight loss, patient activity and progress back to referring health professional • Signposted through other NHS services
Project Monitoring • Surveyed at beginning of programme by the walk leader / cycle trainer on their current travel, confidence when cycling or walking and how motivated they are to be active • Repeated at end of training period, at 3 and 6 months to assess new travel patterns and motivation • Further survey questions (SEGMENT specific) can be included at any time
