  2. n°44UJ5723 Mar. 2005 • Fall from a scaffolding 1.3m • Broken collar-bone • Handrail not fitted, wheels not blocked, personnel not trained (interim)

  3. n°45Sector 7-830 Mar. 2005 • Cutting of a power cable • No serious injury • Task description not completed, personnel not trained, not supervised (interim)

  4. n°46RB 2611 May 2005 • Release of toxic vapour in UX25 • Painting of the guiding line of the MAFI vehicle • Painting done outside the closed perimeter

  5. n°47UA 8330 May 2005 • A person touched the extremity of a powered cable • Non respect of lock-out tag-out rules (consignation) • Reinforcement of safety rules for electrical commissioning

  6. n°48RR 532 Jun. 2005 • Helium release in the LHC tunnel • Injection of helium in a non commissioned line • Line should be isolated when not commissioned

  7. n°49UX457 Jul. 2005 • When removing a 45kg cryogenic safety valve it hit the head of the worker • Broken valve • Lack of competence of the firm, activity not planned, no procedure

  8. n°50UX6522 Aug. 2005 • When cutting a tube the operator got his finger caught • Finger tip cut • Machine is not conform, self certification, similar accident happened before

  9. n°51UX157 Sep. 2005 • Fall when climbing down a scaffolding ladder • Scratch on wrist • Scaffolding not conform, no verification, difficult access, climbing down the ladder the wrong way round

  10. n°52US2521 Sep. 2005 • A plug of the MAFI vehicle exploded • Destruction of plug • Short circuit in the plug and non functioning of the electrical protections

  11. n°53US2528 Sep. 2005 • When installing a circuit breaker the power came back • No damage • Lock-out tag-out procedure not respected.

  12. n°54US4529 Sep. 2005 • Strong electrical shock when touching cable in the false floor • Electrical shock • Old LEP cables not protected, not grounded. Circuit breaker difficult to find

  13. n°55SD186 Oct. 2005 • When pulling cable in PM18, the cable slipped in the machine and the drum came off its axle • Door of SD18 damaged • Inadequate machine for the work being performed

  14. n°56Sector 4-522 Nov. 2005 • A finger got caught when centring manually the EU on the OTU • Finger crushed • Procedure not followed, the correct tool at the work place

  15. Other Accidents • Fatal Accident in RR13, • 2 fingers cut in RE62, • Fingers between soldering machine and support, • Fingers squashed by trailer wheel, • Etc.

  16. 45 accidents or incidents were declared in 2005, 14 were followed by an inquiry, and probably many more were not declared. • Manual handling, falls, incidents or accident of electrical origin, etc. • Procedures badly applied, changed at the last minute, not followed or non existing. • Personnel involved: sub-contractors, interim or experts who are over-confident.

  17. Recurrent Problems • PPSPS not done, or at the last minute, generic, copy-paste, not adapted to the situation • Works not declared, or continuing beyond what is declared or authorised • Safety perimeters not respected • Lack of supervision and preparation • No smoking and personal protection

  18. Safety coordination procedure • PGC - contractual document • Bidders’ conference - rules are presented • PPSPS - 30 days before start of work with precise and full task description • Work Package Analysis, experimental zones • Joint inspection (VIC) - one week before start of work

  19. Work at height • Accident of the Queen Mary in St. Nazaire: 15 dead (Endel) • Decree of Sept. 2004 : Scaffolding, platforms, work at height • Structure calculations, qualification of personnel (erection, control, etc.), training, work with harness, etc. • New European rules in 2006

  20. Working rules for firms • Night and Sunday work is forbidden in France and Switzerland • Derogation only for safety reason (transport, X-rays) or continuity of work (Site guards) • Strictly forbidden to lend personnel (CERN, company, collaboration, etc.)

  21. Consequences of an accident at SLAC • Electrical arc when installing a circuit breaker in an electrical cabinet. • 2nd and 3rd degree burn of a person from a outside sub-contractor. • Stop of all activities during a month and all accelerators for 6 months.

  22. Gravity of faults MANQUEMENT DELIBERE Gravity MANQUEMENT NEGLIGENCE INATTENTION IMPRUDENCE MALADRESSE Sentence

  23. Gravity of faults LE DELIT DE MISE EN DANGER DE LA PERSONNE Expose directly somebody to an immediate risk of death or wound which could be invalidating or mutilating. Example 1: work at height without collective or individual protection. Example 2: work on a machine or process without protection. Example 3: use of dangerous substances without adequate protection Example 4: use of a faulty device

  24. Line of responsibility The line of responsibility IS the hierarchical line Director General Department Leader Group Leader Project Engineer • Note that the Safety Commission, DSO, TSO, etc. are not in the line

  25. Integration of safety • Schedule, Quality, Safety • Optimisation of the installation and maintenance at the design level • Take into account the work environment • Work not well prepared or difficult = loss of time, quality and risk of an accident. • ISO 9000, ISO 14000, OSHAS 18000

  26. Reinforced Safety Coordination Team • 3 safety coordinators (Jean Weber, John Etheridge, Emmanuel Paulat) • 3 more since Jan. 2006 (Hervé Buret, Patrick Thévenon, X) • 1 more Safety Engineer starting Mar. 2006(Stefano Fratianni)

  27. Conclusion • An accident may have catastrophic consequences on the whole project • The level of safety revels the respect of the schedule, of quality and of the environment • Everybody shall be involved • Compression of the planning must be done without compromise on safety

  28. No project is worth the life of somebody
