  13. n°55SD186 Oct. 2005 • When pulling cable in PM18, the cable slipped in the machine and the drum came off its axle • Door of SD18 damaged • Inadequate machine for the work being performed

  15. Other Accidents • Fatal Accident in RR13, • 2 fingers cut in RE62, • Fingers between soldering machine and support, • Fingers squashed by trailer wheel, • Etc.

  16. 45 accidents or incidents were declared in 2005, 14 were followed by an inquiry, and probably many more were not declared. • Manual handling, falls, incidents or accident of electrical origin, etc. • Procedures badly applied, changed at the last minute, not followed or non existing. • Personnel involved: sub-contractors, interim or experts who are over-confident.

  17. Recurrent Problems • PPSPS not done, or at the last minute, generic, copy-paste, not adapted to the situation • Works not declared, or continuing beyond what is declared or authorised • Safety perimeters not respected • Lack of supervision and preparation • No smoking and personal protection

  18. Safety coordination procedure • PGC - contractual document • Bidders’ conference - rules are presented • PPSPS - 30 days before start of work with precise and full task description • Work Package Analysis, experimental zones • Joint inspection (VIC) - one week before start of work

  19. Work at height • Accident of the Queen Mary in St. Nazaire: 15 dead (Endel) • Decree of Sept. 2004 : Scaffolding, platforms, work at height • Structure calculations, qualification of personnel (erection, control, etc.), training, work with harness, etc. • New European rules in 2006

  20. Working rules for firms • Night and Sunday work is forbidden in France and Switzerland • Derogation only for safety reason (transport, X-rays) or continuity of work (Site guards) • Strictly forbidden to lend personnel (CERN, company, collaboration, etc.)

  21. Consequences of an accident at SLAC • Electrical arc when installing a circuit breaker in an electrical cabinet. • 2nd and 3rd degree burn of a person from a outside sub-contractor. • Stop of all activities during a month and all accelerators for 6 months.

Example 1: work at height without collective or individual protection. Example 2: work on a machine or process without protection. Example 3: use of dangerous substances without adequate protection Example 4: use of a faulty device

  25. Integration of safety • Schedule, Quality, Safety • Optimisation of the installation and maintenance at the design level • Take into account the work environment • Work not well prepared or difficult = loss of time, quality and risk of an accident. • ISO 9000, ISO 14000, OSHAS 18000

  27. Conclusion • An accident may have catastrophic consequences on the whole project • The level of safety revels the respect of the schedule, of quality and of the environment • Everybody shall be involved • Compression of the planning must be done without compromise on safety

  28. No project is worth the life of somebody
