  3. Guardianship • ALL Meetings – whether P/T Conferences, Meetings, and/or S-Teams MUST BE HELD with the person who has GUARDIANSHIP!!! • This is not necessarily the person the student lives with.

  4. Vision & Hearing * Must be LESS than 1 year Current • Current • Passed

  5. Attendance • Must NOT be PROBLEMATIC • You need an Attendance Report

  6. Progress Monitoring • 10 Weeks of Data in Reading / Math Data • AimsWeb (elementary) • Reading Plus (middle school) • Stanford Math (elementary & middle)

  7. What to Submit • 1) Attendance Report • 2) VI / Hear Reports • 3) Teacher Referral • 4) Progress Monitoring • 5) What other interventions have been attempted to make this student successful.

  8. School Counselors • Copy of Final S-Team Teacher Referral Form • Copy of Progress Monitoring Data • Parent’s Invitation to Meeting Letter • Confirmation of S-Team Meeting

  9. S-Team Meeting • Parents • Classroom Teacher • Special Ed Teacher • Counselor • Psychologist / Social Worker • Administrator
