  4. The HP Tool Kit • Medication Use Guidelines (MUGs) • Evidence-based, standards of care • Contain medications that target end of life symptoms • Contain protocols/algorithms derived from palliative care literature

The HP Tool Kit • Per Diem • MUG medications related to a patient’s terminal diagnosis are included under the per diem • Medications not included on the MUGs are billed outside of the hospice per diem • Quick Guide to Services HP Pharmacists are able to assist you to create the most appropriate medication care regimen for your patients!

Medication Access • Routine medication • HP dispenses hospice medications 95% of the time • Urgent medication needs • Local Pharmacy Network is utilized for emergency medications • 4-day supply of MUG, diagnosis related medications • HP Prescription Card administered through Scrip Solutions (remains active as long as the patient is on HP’s service)

HP’s Automated Systems • Automated Refill & Discharge System—available via the HP toll-free phone number 1-877-882-7820 • Xeris--available via the HP website www.hospicepharmacia.com • These systems allow hospices to: • Admit patients • Access medication profiles • Perform refills • Track packages and patient assessments • Discharge patients and medications
