Status • Accident has occurred • Machine is down • Work is suspended • Type A investigation has started • Estimated duration: 3 weeks • Interviews, assessment of safety • knowledge, implementation, culture, attitude • Safety must come first • Must change our culture • Safety must come first • Generate ideas and steps to take charge of the safety culture in our group and in our programs • Suspension removal incremental • Depends on progress made towards changing culture • Gunther will meet daily with research division head • Result is daily update what is permitted and what not

  4. Directors Message • Message from Jonathan October 18, 2004 • As you all know, we had a very serious accident at SLAC last week involving an electrician. We re-extend our heartfelt concern to his family and continue to pray for his rapid and full recovery. • The rationale for the current work suspension is neither to assert, nor to imply, blame to any one of you for this accident. That is not my aim, nor would it be appropriate. Specifics • ES&H good practices • Already shown relevant slides of most relevant ES&H Safety Manual last week: • http://www-group.slac.stanford.edu/esh/manuals/eshmanual.html • Medical • Hazards • Evacuation • Electrical • Since most people work around electrical/electronics equipment and won’t be able to take class in the next couple of weeks. • Went over relevant slides from Electrical Safety for R&D equipment • When suspension of work is over • Clarify that exposure to above 50V need Gunther’s assessment that it is < 5mA/10J before proceeding. • If above 5mA/10J, need Hot Work permit. • Identify and mitigate risk of exceptions (if any). (e.g. insulate potentially exposed contacts, etc).
