  3. Case study • 80 year old woman • COPD, MI, anxiety • BMI 13.5 • Lives alone, community alarm and good family support • Frequent admissions to Ninewells – admissions during OOH for increasing symptoms- anxiety potential contributing factor

  4. Case study • MDT involving Respiratory Consultant, RLN, Community dietician, Community Resp Nurse and Occupational Therapist • Able to review and optimise treatment • Opportunity for communication between community and acute.

  5. Outcomes from MDT • Arranged period of respite • OOH plan for community alarm staff • Further anxiety management and sessions • Restarting of care support • Ongoing support from Community Respiratory Nurse and Dietician
