  1. Viscodissection - as safe as possible - Dzinic V, Dzinic M Private Eye Clinic “Prof dr Dzinic” "Disclosure statement: no conflict of interest"

  2. Purpose: to show the way of preventing posterior capsule rupture during pahacoemulsification chop technique in patients with different type of cataract maturity.

  3. Material and methods: 60 patients (74 eyes) undertaken cataract surgery; Visual acuity from L+P+ to 0.6 /Snellen chart/; Nuclear hardness from N+1 – N +4; Coaxial 2.75mm phaco, temporal approach, horizontal chopping technique; Capsulorexsis from 6,5-7,5mm;

Material and methods: Hydro dissection was preformed, viscoelastic /Amvisc Plus / was introduced in the same way as hydro dissection until slight elevation of lens was achieved ; Phaco chop and in the bag implantation /41 eyes received single piece hydrophilic acrylic lens and 33 eyes three piece silicone lens/:

Results: In all patients uncomplicated cataract surgery was performed; Best corrected visual acuity during follow up period was 0.8-1.0; Introducing viscoelastic beneath lens provide cushion between the lens and posterior capsule, providing displacement of posterior capsule and preventing rupture; In the same time light elevation of the lens in conjunction with slightly larger capsulorexis provides easier placement of the chopper:

Conclusion: Study showed that technique of viscodissection was very useful in preventing posterior capsule rupture without any side effects; Despite larger rexis and different kind of lenses size that were implanted, they were all stable in the bag and good visual rehabilitation was achieved during follow up period.
