AWARDS… • Designed to provide formal recognition for outstanding accomplishments and performance to the mission of the PHS or other programs • Officers DO NOT compete against each other • There are no minimum or maximum number of nominations • Open to all officers on active duty (Reserve and Regular Corps and All Categories and Grades)

  5. Processing… • 2000 – 121 individual • 2002 – 153 individual • 2004 – 264 individual • Increase – 118% • 2000 – 245 unit • 2002 – 360 unit • 2004 – 641 unit • Increase – 162%

SERVICE AWARDS (2 of 5) • National Emergency Preparedness Award • Service in an approved activity (CCRF/D-MAT/ NDMS/OEP) minimum 2 continuous years • After fully trained and deployable. • If earned the Field Medical Readiness Badge (FMRB) through OFRD you qualify • Bicentennial Unit Commendation • 1998 to commemorate the 200th anniversary of the creation of the USPHS.

  11. SERVICE AWARDS (3 of 5) • Hazardous Duty Service Award • 180 Days or more in designated area • Temporary or permanent assignment • Not training • Foreign Duty Service Award • 30 consecutive days or 90 non-consecutive days • Temporary or permanent assignment • Not training

SERVICE AWARDS (4 of 5) • Special Assignment Service Award • Detailed 30 consecutive days or more to other federal or state agency • 30 non-consecutive days to the same agency for an on-going activity. • Assignments special initiatives of the SG including CPO or PAC member. • Crisis Response Service Award • Direct hands-on service for 7 days at the site of a problem, SG declared a crisis.

SERVICE AWARDS (5 of 5) • Isolated Hardship Service Award • 180 Days or more in designated area • Temporary or permanent assignment • Not training • Smallpox Eradication Campaign • Served 90 days or more cumulative service to CDC Bureau of Smallpox Eradication or Smallpox Laboratory from Jan.1, 1966 to Oct.26, 1977.

  14. CASH AWARDS • PHS commissioned officers are NOT ELIGIBLE for performance-based cash awards. • They are ELIGIBLE to receive cash awards for suggestions, inventions, and scientific achievement outside the scope of their duties.

  28. Reasons Awards are not Endorsed • Officer nominated for incorrect award • Untimely award submission • More than 1 individual award for per award cycle year • Nomination forms filled out incorrectly or have administrative errors • Award narratives are poorly written

  29. Narrative Pitfalls (1 of 2) • Poorly written, unfocussed, unclear • Incorrect grammar, punctuation, abbreviations • Narrative doesn’t justify the level of award

  30. Narrative Pitfalls (2 of 2) • Accomplishment is not yet completed • Accomplishment was already recognized • Time period covered does not match PHS 6342

LIMITATIONS… • New awards cannot be based on old awards, however, a previous award can serve as part of a more senior award based on significantlygreaterachievement over a longerperiod of time. • No officer can receive two individual honorawards in one award cycle (July 1 - June 30). • Awards cannot be for an ongoing activity, it must be completed to show impact
