Venous Thromboembolism (VTE) Committee Automated VTE Report PROMPT Screen • Output to File/Printer/MINE- “MINE” will return report to screen, otherwise, select printer • Select Facility from List- Select valid Facility to qualify • REPORT DETAIL section on report • The patient population will be Registered to the facility selected above • Inpatient visit status only (NOT Observation, ED, Day stay or etc.) • 18 years or older • Will then evaluate to see if patient does NOT have an active order • (means ordered status not complete/discontinued) for any variation of following: • Argatroban Enoxaparin • Fondaparinux Warfarin • Heparin (except heparin flushes) • SCD hose TED hose • If patient does not have ANY of the above, then will display on report. • Will include LAST result only for Hemoglobin, Hematocrit, INR and Platelets • Will page break by nurse unit and sort by Admit date/time and Account#

  6. Venous Thromboembolism (VTE) Committee VTE System Report 1. Admission Dt/Tm – The Date/Time the patient was admitted as Inpatient Status 2. Acct # - The account number for visit that qualified 3. Patient Name – The name of the patient 4. Admitting Physician – The physician that has active ‘Admitting’ relationship with the patient 5. Hemoglobin/Hemoglobin Dt/Tm - The LATEST result for Hemoglobin and the Date/Time of that result 6. Hematocrit/Hematocrit Dt/Tm - The LATEST result for Hematocrit and the Date/Time of that result 7. INR/INR Dt/Tm - The LATEST result for INR and the Date/Time of that result 8. Platelets/Platelets Dt/Tm - The LATEST result for Platelets and the Date/Time of that result

EMR Process for VTE Report Print 0700 daily in the pharmacy of each facility Will print one unit per page Pharmacist review the e-Mar to determine if the patient is actually on VTE prophylactic (7 days a week) Pharmacist will notify the attending physician the patient is not on VTE prophylactic Attending physician will order VTE Prophylactic or document the reason why patient is not on VTE Prophylactic Paper Process for VTE Report Print at 0700 daily to Pharmacy Satellite on Tower 4 Will print one unit per page Pharmacy will take the report to each designated Unit & give it to the APP/Charge nurse (7 days a week) APP/Charge nurse will contact staff for follow up w/attending physician Attending physician will order VTE Prophylactic or document the reason why patient is not on VTE Prophylactic Venous Thromboembolism (VTE) Committee VTE Process Flow
