Data Collection Rules • All information you base your option selection on for each element in the MCMP-PAT must be documented in the medical record or other clinical resource you have available at the point of care, i.e., a registry. • When collecting data, follow these steps: • Determine if the patient received the therapy or service, e.g., a drug or laboratory test • If they did not, determine if there are medical, patient or system reasons for not receiving the therapy/service where applicable • If there are no reasons found, select the “no/no reason documented” option 17
Data Collection Rules (cont.) • Example: The documentation in the med. rec. does not have to contain verbiage that states the reason for not prescribing an antiplatelet is because the patient has a bleeding disorder. You can select the “Not prescribed for medical reasons” without this direct linkage spelled out in the documentation. All you need is the bleeding disorder mentioned in an H&P or progress note, etc. • Example: You do need to have the direct linkage spelled out in the documentation if you select the “Not prescribed for medical reasons” because you are using the “other reason documented by the practitioner for not prescribing aspirin or clopidogrel” • Documentation supporting a direct linkage is also required when you select “Not prescribed for patient reasons” or “Not prescribed for system reasons” 18
Data Collection Rules (cont.) • For all measures you cannot collect information more recent than that dated the last day of the measurement period (6/30/09) • When abstracting the confirmation of a diagnosis (HF, DM, CAD, or 1 of the chronic conditions for PC) remember to follow the definitions for each option: • No/No reason documented: Select this option if the patient has no documented history of ? anywhere in the office/clinic record. • Medical record not found: Select this option if you are unable to find the patient’s medical record. 19
Data Collection Rules (cont.) • Confirmation element (cont.) • The DM module has an additional option called Not Included for Medical Reasons which is selected if the patient has polycystic ovaries (history of), gestational diabetes (measurement period/year prior) or steroid induced diabetes (measurement period/year prior). • Measurement period = 7/1/08 – 6/30/09 • Year prior = 7/1/07 – 6/30/08 • “History of” means any time in the patient’s history up through the last day of the measurement period (6/30/09). 20
Lessons Learned from PY1 Data Collection & Audit • Continues to be confusion over definitions of options in the confirmation element, i.e., “skip – rec. not found” vs. “no” so “skip – rec. not found” verbiage modified to “medical record not found” • Emphasis on the processes related to claims-based measure collection – now noted in the option as “claim not found” instead of “skip” • Additional clarity needed on purpose and use of denominator exclusions • “Yes” option vs. “Not prescribed for patient reasons” • Between types of exclusions, i.e., patient vs. medical exclusion • Attention to timeframes is important, i.e., pneumonia vaccination given after the end of the measurement period would not be answered “as given” 21
Lessons Learned from PY1 Data Collection & Audit • Measures with a result and a date the laboratory test was drawn need to be paired • “History of” was clarified when used to determine where in the medical record documentation may be retrieved from • Determination of ‘invalid visits’ 22
