Benign biliary stricture • Symptoms • pyrexia, chilling, abdominalgia and jaundis is caused by repeated imflammation • 시술 후 경과,예후가 좋기때문에 적극적인 치료를 요하는 질환

PURPOSE • 2004년 3월~2005년 1월까지 담도결석으로내원한 환자중 • Benign bile duct stricture가 동반된 17명의환자에게, • Retrievable PTFE-coverd stent를 6주동안 설치해 예후를 살펴, • Retrievable PTFE-coverd stent의 유용성에대한 고찰을 하기 위함.

Stent insertion • 거의 대부분 담석제거술을 시행하던 환자중, • 환자의 PTBD tract을 통해 drainage tube제거 후, • 10F sheath를 유치시킨 후Stent insertion • Stent insertion후 post balloon dilatation시행 • Drainage tube 유지

Stent removal • Sheath 끝을 구별할 수 있는 재료사용 • Retrieval hook를 이용해 스텐트 proximal 부위의 실을걸어 sheath안으로 잡아당김 • Bile duct를 통해 stent removal

Stent migration • 17명의 환자 중 2명 migration • 병변부위에 hook or balloon으로 재설치 • Migration환자는 2주동안 stent keeping • 2주 후 cholangiography 실시 후 • Stent removal

Consideration • Benign biliary stricture는 시술결과 좋은 예후를 보였다. • 그러나 6주 동안만의 결과만 얻은 것이기 때문에 • 좀 더 긴시간의 예후를 살펴보는 것이 바람직하다고 사료된다.
