Operational Standards and Procedures • All Network Standards, Procedures, Policies, and Network Databases must be developed prior to first equipment deployment. • Equipment must be designed, engineered, and installed using the developed standards and procedures. • Must be scalable and able to grow from system 1 to system 200+ • Are required so that all users (customers/vendors/operators) conform to a single network philosophy • Will drive work flows for all on-going operations • Will require at least 4 to 5 months to develop (List of items on following pages are meant to be directional and not all inclusive) • Information has to be packaged, signed off on, and shared with Vendors and Customers prior to the first customer system deployment

Back Office System Standards and Procedures • Network Management system (NMS if needed) • Billing • New Fiber Sales • Yearly/Quarterly O&M • Per Call/Per Hour (if applicable) • Provisioning • Fiber Assignments • Capacity Management • Rack/Shelf Space • Management Network (see ISP standards) • Element Management Systems • Connectivity and management architecture • Per Vendor tied into Master NOC • Connections to Vendor/Customer NOCs • Trouble Ticketing • Call/Event Logging • Notification Systems and Schedules • Escalation Procedures • Standard Maintenance • GIS/Fiber Mapping with Call Before You Dig tie ins • Reporting Methodology – Monthly/Quarterly to DF customers? • Outages • Tickets/Events • Data Storage • Test Results • Standards Documentation • Authorized Companies and Personnel • Shipping Addresses per site • As-Builts for Fiber, Equipment, and Facilities • ROW & Permit agreements • Tax records
