ELR CCG • Clinical leadership from Managing Director and GP Board Members • In addition our Board benefits from a secondary care clinician, Board Nurse, Independent Lay Members and CCG managers • Assessed as ready to take on statutory duties with conditions from April 2013 after rigorous national process • Anticipate that these conditions will be removed in April • We have been listening, involving and relationship building from the outset

  3. Our patch • 315,000 people • 34 GP practices • 3 localities • 5 community hospitals • 2 large providers • 2 local authorities • 4 district authorities

  4. People in our patch • Better than average health • Affluent, rural patch with areas of deprivation • High and rising elderly population • 103% increase in dementia sufferers by 2030 • 72% of deaths from cancer, CVD and COPD

  5. Our patients and partners said… To improve health by meeting our patients’ needs with high quality and efficient services, led by clinicians and delivered closer to home.

Plans for 2013/14 • Implenting the Integrated Care Model • Long Term Conditions schemes including Diabetes, COPD, Stroke • Improving timely diagnosis of Dementia • Developing initiatives to support end of life care • Enhancing services available in community settings • Improving mental health and Learning Disability care pathways • Completing and consulting on our Minor Injuries Services Review • Improving maternity & paediatric care pathways

  8. What next? We’ll also be focusing on a variety of other projects and services including: • Maternity services • Healthcare services for disabled children • Healthcare services for people with complex and challenging behaviour needs • Develop and strengthen of joint commissioning with local authority partners • Patient transport services • moving settings of care i.e. dermatology, ophthalmology from acute to community settings • Review of Out of Hours (OOHs) • A review of the Individual Funding Requests (IFR)
