How To Order First Health Part D Kits – Step 1 Our user-friendly web-based ordering system makes it simple for you to request First Health Part D sales materials to educate and enroll your clients. In order to access the ordering system, you need to be certified and “ready to sell” First Health Part D and have been assigned an Agent Writing Number (AWN). Important Note: 2013 Sales Kits not available on this site after 10/31/2013 . Contact Customer Service @ 866-714-9301 if needed after this date. Step 1 From our Broker Portal, click on: Order 2013Sales Kit for Part Dor Order 2014Sales Kit for Part D to begin.

  3. How To Order First Health Part D Kits – Steps 2 and 3 Step 2 Enter your assigned 6-digit Agent Writing Number (AWN) to gain access to the ordering area of the site. Step 3 Click the box titled Continue.

  6. First Health Part D Kit Allocations Kit allocations are based on previous selling history and a kit-to-sale ratio of 5:1. For example,if during 2013 you submitted 9 Coventry PDP applications, your kit allocation for 2014 AEP would be 45 (9 sales times 5 = 45). That allocation is then broken out by the two and a half month AEP season, October 1 – December 7, so you could order up to 18 kits in October, 18 kits in November and 9 kits in December. Unused monthly kit allocations will carry over. For new agents, there is a default kit allocation of 15 kits per month. IMPORTANT NOTE: If you need additional kits due to new sales opportunities, click on the Help button in the upper right corner. You will need to send an email with your request to: PDPkitrequests@cvty.com. Click on FH14PEBKfor your monthly kit allocations.

  7. How To Order First Health Part D Kits – Step 6 and 7 Step 6 After adding the Qty*, then Click on Checkout Step 7 Next, you need to check the Select box next to the item you wish to order. Also, in the Qty* field, enter the amount of kits you want to order.

Step 10 Enter your AHIP Certification Code and DistributionPartner Name (these are optional fields.) Step 12 Select Shipping Information. TIP All shipments will mail Standard Shipping Method at no cost to you. Should you choose expedited shipping, you will need to enter your credit card information to cover the cost. Step 13 Click on Next.

NOTE: If you have provided a valid email address you will receive an email confirming your order. Once your order has shipped, you will receive another email with tracking information.
