  1. Faculty Guide to Using the OSD Online Database Services Office for Students with Disabilities Launch Date: Spring Quarter, 2012!

The Office for Students with Disabilities is transitioning to online services! • Why now? • Because we want to help facilitate a smoother accommodation process for both Faculty and students, while at the same time going ‘green.’ • How will it work for students? • Students will request all accommodations/exams online and notify their Instructors of such requests to maintain rapport of being registered with OSD. • How it will work for Faculty/Instructors • A 3-step process detailed in this guide. • See it in action!   3. How the OSD Online Database will help Faculty • No more forms! • All Requests, Testing Contracts, and Exams will be submitted online!

What Faculty Need to know • Faculty will receive VARIOUS emails THROUGHOUT THE QUARTER from OSD. • Emails may be: FYI, Need your Approval, Need your Assistance, Need your Action • Check your email at the beginning of each quarter • Create a File Folder on your email for Students who request Accommodations EMAIL TYPES QUICK TIPS

Emails: Notification of Academic Support Services (NASS) • This notice will be sent to faculty when a student is approved for accommodations by OSD, registers for course via GET, AND REQUESTS Accommodations via OSD online services • NASS lets you know what accommodations the student is approved for, any additional notices regarding accessibility, and provides a link to create an “Alternative Testing Contract”

The Alternative Testing Contract • The Alternative Testing Contract link allows Faculty to: • specify the amount of time provided to all students for each type of exam administered throughout the quarter, what is allowed during the exam, and any other pertinent details ALL AT ONCE! • Modifications to the Alternative Testing Contract can be made (must submit request 48 hours prior to testing date by calling the OSD or emailing: OSDExams@cslanet.calstatela.edu).

Alternative Testing Contract Steps • Step One: Click on the Alternative Testing Contract Link (on the NASS email) • Step Two: Select the Alternative Testing Contract Type • Step Three: Answer a few questions and Fill in the blanks, then Click on submit • You have Successfully submitted an Alternative Testing Contract… It’s that easy!

  10. Alternative Testing Contract • Step One: Click on the Alternative Testing Link included in the body of the NASS email (inside red box). Alternative Testing ContractStudent is eligible for Alternative Testing services, please fill out an testing contract for the student by using the following link: https://www.gunadiframework.com/CSULA/ContractInstructor.aspx?ID=233&CID=72936&Key=iRjmUAqZ. If you have filled a similar testing contract for another student in the same class, please do not hesitate to contact us or send us an email to copy the alternative testing contract for the other students.

  12. Select the Alternative Testing Contract Type • Step Two: select the Alternative Testing Contract Type - “ADA Reasonable Accommodation Testing Contract”, • then click on “Continue to Fill Contract”

Fill in the Alternative Testing Contract • Step Three: Specify what is generally allowed for all students such as open notes, note cards, etc • Fill in the time allotted for each type of exam administered throughout the quarter (if you are not administering a specific exam, please leave that field blank) • Provide your phone number and any additional comments • Click on “Submit Alternative Testing Contract”

Modifying an Alternative Testing Contract • Contact OSD at least 48 hours prior to the exam. • We will make the changes to the contract per your request (changes can be made over the phone by calling: (323) 343-3140 or by emailing us at: OSDExams@cslanet.calstatela.edu • All changes will be documented and only AUTHORIZED OSD STAFF will have access to contract and exam information

Uploading Exams • Faculty will receive a “Testing Request Reminder” email with a link to upload exam (48 hours prior to scheduled exam date) • Click on the Link, choose your file, and upload your exam (process works as any other attachment file) • System Update is Successful! • Only AUTHORIZED OSD STAFF will have access to uploaded exams • All uploaded exams will be deleted after use
