2. Jul. 12 – Jul. 18 Kent July 14, 2014 EE operating offline sorter reach across conveyor to stand a downed can and contacted right forearm with driven conveyor belt. EE was given a can stick to avoid reaching across the conveyor.

3. Jul. 12 – Jul. 18 St. Paul July 14, 2014 Contractor was seen attempting to put a can conveying belt back on pulley while machine running. E-stop was immediately pushed. Contractor told to follow Lock/Tag/Verify.

4. Jul. 12 – Jul. 18 Fairfield July 14, 2014 8" riser in stage 2 of Washer blew gasket and shot treated water30' across floor to wall. Pumps and Washer shut off for repairs. Fortunately nobody was walking by when gasket blew.

5. Jul. 12 – Jul. 18 Medical #17 in 2014 July 16, 2014 St. Paul EE was cleaning front end pits, wearing gloves. Getting out, placed right hand on a grating support beam. Piece of aluminum scrap on beam caused a small puncture between his thumb and index finger. EE received 3 sutures & returned to work. Location where puncture occurred

6. Jul. 12 – Jul. 18 Olive Branch July 18, 2014 EE was operating palletizer on L-3, as EE was operating in manual mode it was notice that the sweep would inch along when the light curtain was tripped. EE contacted Supervisor and ET. Program changed to stop machine when curtain was tripped.
