Advanced FellowshipsOutcomes • AF has graduated over 2000 fellows since its inception in 1993. • Portfolio now trains approximately 50% physicians and 50% associated health professionals. • Graduates of the program have gone on to assume major leadership roles in VHA, Dept. of Defense, and Academic Medical Centers. • Leadership opportunities must be tailored to your (the individual fellow’s) interests and goals. • Leadership may relate to clinical practice, research, education, administration, and/or policy.

Centers of Excellence in Primary Care Education Vision: • Veterans, families and caregivers are vital partners! • Trainees learn to provide outstanding team-based primary care • Interprofessional and collaborative academic and practice teams share continuous learning goals • Work influences national policies and practice Purpose: Foster transformation of clinical education by preparing graduates of health professional programs to work in and lead and improve patient‐centered interprofessional teams that provide coordinated longitudinal care. Developed in response to VA’s rapidly changing clinical system of care
