Clinical disorders • “-osis” • Arterial pH를 변화시키는 any pathologic process • Lower pH  acidosis • Increase pH  alkalosis • Metabolic : primarily affects [HCO3-] • Respiratory : primarily affects PaCO2 • Secondary compensatory response : not “-osis” • Eg. Metabolic acidosis with respiratory compensation • “-emia” • All primaty process와 compensation의 net effect • pH < 7.35  acidemia • pH > 7.35  alkalemia

Compensatory mechanism Physiologic response to change in [H+] 1. immediate chemical buffering 2. respiratory compensation 3. slower but more effective renal compensation 1. body buffers • Bicarbonate : H2CO3/HCO3- • Hemoglobin : HbH/Hb- • Other intracellular protein : PrH/Pr-, polypeptide • Phosphate : H2PO4-/HPO42- • Ammonia : NH3/NH4+ • Bicarbonate buffer • ECF에 high conc.으로 존재, PaCO2와 함께 lung과 kidney에서 엄격히 조절됨으로 중요 • Repiratory disturbance보다는 metabolic disturbance에 effective • PaCO2증가에 effective하지 않고 respiratory acidosis의 severity를 반영하지 못함 • Hemoglobin buffer • Rich in histidine  effective buffer from pH 5.7~7.7 • ECF의 가장 중요한 noncarbonic buffer • CO2(volatile acid)와 noncarbonic acid(non-volatile acid) 모두 buffering할 수 있음

Pulmonary compensation • Brain stem의 chemoreceptor에 의해 mediate : CSF의 pH변화에 반응 • PaCO2 1 mmHg 증가 시  minute ventilation이 1-4 L/min 증가 • Metabolic acidosis시 • Blood pH감소  medullary respiratory center를 자극  alveolar ventilation증가  lower PaCO2  restore aretrial pH toward normal • [HCO3-]가 1mEq/L 감소할 때마다 PaCO2가 1-1.5 mmHg 감소하게 됨 • Metabolic alkalosis시 • Arterial pH증가  depress respiratory center  alveolar hypoventilation  elevate PaCO2  restore pH • 이로 인한 Hypoxemia는 결국 oxygen sensitive chemoreceptor를 activation시키게 되어 compensatory pulmonary response는 제한점이 있음 : metabolic alkalosis시 PaCO2가 55mmHg 이상은 오르지 않음 • [HCO3-]가 1mEq/L 증가할 때마다 PaCO2가 0.25-1 mmHg 증가

Renal compensationDuring acidosis (1) Filtered HCO3-의 재흡수 증가 - renal tubular cell 안의 CO2가 water와 결합하여 carbonic anhydrase에 의해 carbonic acid를 형성. - rapidly dissociate into H+ and HCO3- - bicarbonate(HCO3-)는 blood stream안으로 들어가고 H+은 renal tubule로 분비됨 - H+는 filtered HCO3-와 반응하여 다시 water와 CO2형성 diffuse back into renal tubular cell Porx. Tubule : filtered bicarbonate의 80-90% 를 재흡수 Dis. Tubule : 나머지 10-20% 재흡수

  7. Compensatory mechanism3. Renal compensationDuring acidosis (2) Titratable acid의 배설 증가 - tubular fluid의 HCO3-이 모두 교정된 후에는 배설된 H+은 HPO42-와 결합하여 H2PO4-를 형성 - H2PO4-는 charge때문에 재흡수 되지 못하고 urine으로 배설되게 됨 - net result  H+은 몸밖으로 배설되고 HCO3-는 blood stream으로 들어가게 됨

Renal compensationDuring acidosis (3) Ammonia 생성의 증가 - HCO3-가 완전히 재흡수 되고 phosphate buffer가 소비된 후에는 NH3/NH4+ pair buffer가 가장 중요한 urinary buffer가 됨 - acidemia시 renal NH3 production이 현저히 증가 - renal tubule로 나가 H+과 결합하여 NH4+을 형성하게 됨 - NH4+는 luminal membrane을 통과하지 못하므로 urine으로 배설되어 결국 H+의 제거가 이루어짐

Renal compensation During alkaosis • Kidney는 엄청난 양의 HCO3-를 배설할 수 있어 metabolic alkalosis에 대해서는 highly effective • 따라서 metabolic alkalosis는 주로 concomitant sodium deficiency나 mineralocorticoid excess와 같은 상황에서 생기게 됨 Base excess • Full O2 saturation과 37°C하에서 Blood pH를 7.40으로, PaCO2를 40mmHg로 만드는데 필요한 acid나 base의 양 • 측정된 완충염기에서 정상완충염기 양을 뺀 값 • positive value시 : metabolic alkalosis • negatuve value시 : metabolic acidosis

Physiologic effect of acidemia • Direct myocardial and smooth muscle depression  reduce cardiac contractility and peripheral vascular resistence  progressive hypotension  Tissue hypoxia • Endogenous and exogenous catecholamine에 대한 반응 감소 • Ventricular fibrillation의 역치 감소 • Progressive hyperkalemia (pH 0.1감소 = K 0.6증가) • CNS depression : metabolic 보다 respiratoryacidosis에서 prominent (= CO2 narcosis : increased cerebral blood flow로 인한 intracranial hypertension과 severe intracellular acidosis의 결과)

  12. 2. Respiratory acidosis • Acute respiratory acidosis • 보상기전이 제한적 • PaCO2 매 10mmHg 증가시 마다 HCO3 1mEq/L증가 • Chronic respiratory acidosis • 12~24 시간 지난 후부터 renal compensation 의미있음 • 3~5일 후 peak • PaCO2 매 10mmHg 증가시 마다 HCO3 4mEq/L증가

Respiratory acidosis • Treatment • 대부분 By increasing alveolar ventilation • Reducing CO2 production only in • Danrolene for malignant hyperthermia • Muscle paralysis for tetanus • Antithyroid medication for thyroid storm • Improving alveolar ventilation • Bronchodilation • Reversal of narcosis • Doxapram: respiratory stimulant • Diuresis : improving lung compliance • Mechanical ventilation의 indication • Moderate to severe acidosis (pH<7.20) • CO2 narcosis • Impending respiratory muscle fatigue • IV NaHCO3 • Rarely necessary unless pH < 7.10 and HCO3- < 15 mEq/L • Transiently increase PaCO2 • Chronic baseline respiratory acidosis환자에서 산소 치료 시 주의 • Respiratory drive가 PaCO2가 아닌 hypoxemia 에 의존하기 때문

Metabolic acidosis • Primary decrease in HCO3 • Cause • HCO3 consumption by strong nonvolatile acid • Renal or GI wastingof HCO3 • ECF에서의 rapid dilution with bicarbonate-free fluid • Anion Gap • [Na+] + unmeasured cation = [Cl-] + [HCO3-] + unmeasured anion • AG = unmeasured anions – unmeasured cations = [Na+] – ( [Cl-]+[HCO3-] ) • Normal 7~14 mEq/L

Metabolic acidosis • Treatment • Respiration should be controlled • pH 7.2이하로 계속된다면 alkali therapy고려(NaHCO3) • Amount : empirically as a fixed dose(1 mEq/kg) or base excess와 calculated bicarbonate space로 계산 • Bicarbonate space • HCO3를 정맥으로 주었을 때 bicarbonate가 분포하게 되는 volume • NaHCO3 = Base Deficit x 30% x body weight in L (bicarbonate space =30%) • pH가 > 7.25 가 되면 adverse physiologic effects를 극복하기에 충분 • Profound or refractory acidemia시 dialysis가 필요할 수도 • Cardiac arrest나 low flow state에서 large amount of NaHCO3의 routine use는 더 이상 추천되지 않음 • Paradoxical intracellular acidosis가 생길 수 있기 때문(CO2는 cell안으로 들어가지만 bicarbonate는 들어가지 않기 때문)

Metabolic acidosis • Treatment • Diabetic ketoacidosis시 • Hyperglycemic osmotic diuresis로 인한 existing fluid deficit의 Replacement, insulin, potassium, phosphate, magnesium • Lactic acidosis시 • Adequate oxygenation and perfusion의 회복이 일차적 목표 • Salicylate poisoning시 • NaHCO3로 urine alkalization시켜 salicylate elimination 증가 시킴 • Methanol or ethylene glycol intoxication시 • Ethanol infusion으로 alcohol dehydrogenase의 경쟁적 방해

Anesthetic consideration, acidosis • Acidemia는 대부분의 sedatives and anesthetic agents의 CNS and circulatory system에 대한 depressant effect를 potentiation • Most opioids는 weak base로서, acidosis시 opioid 의nonionized form이 증가되므로 opioids의 brain통과 촉진 • Increased sedation과 airway reflex의 감소는 pulmonary aspiration에 걸리기 쉽게 함 • Volatile and IV anesthetics의 circulatory depressant effects 강화 • Hyperkalemia 동반된 acidotic pt.에서 succinylcholine은 피해야 함.

Physiologic effect of alkalosis • Hb.의 oxygen에 대한 affinity가 증가되어(left shift) tissue로의 산소공급이 어려워짐 • K+이 cell안으로 이동하여 Hypokalemia 가 발생할 수 있음 • Ionized plasma [Ca2+]이 증가하여 circulatory depression and neuromuscular irritability초래 가능 • Respiratory alkalosis • Reduce cerebral blood flow • Increase systemic vascular resistence • Coronary vasospasm 촉진 가능 • Bronchoconstriction • Decrease pulmonary vascular resistence

Repiratory alkalosis • Primary decrease in CO2 • Increased ventilation compared to increase in CO2 production • PaCO2 10mmHg => HCO3 2mEq/L • Variable compensation during chronic respiratory alkalosis • Treatment • Underlying process correction • Severe alkalemia시 (pH > 7.60) • IV hydrochloric acid • arginine chloride • ammonium chloride

Metabolic alkalosis Chloride sensitive, chloride resistant • Chloride sensitive • ECF volume depletion • Sodium reabsorption 증가 • Electronneutrality • Chloride 소모 • H+ excretion • HCO3 재흡수 증가 • K 배설 증가  hypokalemia Chloride Sensitive Metabolic Alkalosis • Chloride resistant • Mineralocorticoid activity 증가

Metabolic alkalosis • Treatment • Chloride sensitive시 TOC • Iv saline(NaCl) and potassium • Gastric fluid loss가 원인일 때 H2-blocker가 유용 • Edematous pt.에서 acetazolamide가 유용 • Mineralocorticoid activity증가와 연관시 • Aldosterone antagonists(spironolatone) • pH가 7.6이상시 • IV hydrochloric acid, ammonium chloride, arginine chloride or hemodialysis고려

Anesthetic consideration • Opioid induced respiratory depression의 기간 연장 : opioid의 protein binding증가 때문 • Cerebral ischemia가 생길 수 있음 • 특히 Hypotension시 respiratory alkalosis로 인한 CBF의 현저한 감소 시 • Hypokalemia 와 combination 될 경우 atrial and ventricular arrhythmia가 생길 수 있음. • Non-depolarizing neuromuscular blockade를 potentiation
