  2. 2012 Information and Changes Full-time employee health premiums will stay the same for 2012. HMO OPTIONS ARE CHANGING! Medco will be the new pharmacy manager for all health plans. Long-term disability changes and special open enrollment period. Increased rates for some dental plans. State open enrollment: November 7 – November 18.

HMO Options are Changing! • One HMO per county as opposed to multiple HMO providers. • AvMed is the designated HMO for UNF area counties. • United HealthCare participants need to actively select AvMed during open enrollment or will be defaulted into the State PPO Plan through BlueCross BlueShield. • Any changes to this information will be communicated immediately.

Pharmacy Benefits Manager • Medco will be the new pharmacy manager for both the PPO and HMO plans. • PPO members will still be required to use mail order for maintenance prescriptions. • Medco will send a letter with detailed information and new cards. • Walgreens is NOT in-network with Medco.

  5. Long-Term Disability • New Vendor: The Standard • Enhanced benefits with slightly lower rates. • Guaranteed Issue. No medical application or review required during open enrollment period. • Special open enrollment period October 3, 2011 – December 1, 2011. • Contact: Stan Miller (904) 536-5337 or the Office of Human Resources.

  7. 2012 Plan Year Open Enrollment • Open enrollment dates have changed! • November 7, 2011 – November 18, 2011. • Only two weeks to make changes to state plans! • Special LTD open enrollment is longer: October 3, 2001 – December 1, 2011.
