Expectations for the provision of medicines CASE: Thursday the 1st of March 2007, a nurse at a medical centre caring for the elderly requested my advice as a consultant pharmacist about a prescribed analgesic treatment for an 83 year old woman approaching her life’s end. Her attending physician was unavailable. The patient was highly agitated despite the use of buprenorphine 52.5 mcg/h patches together with a reserve medication of morphine 10mg injections twice daily. This medical regime being inadequate, I proposed an additional half patch and the use of Temgesic instead of morphine which was unavailable both in the nursing home and in my pharmacy. My decision having been accepted by the physician on duty, the order was given to the wholesaler at 5.30 pm and I received the medication the same evening at 10.30pm, I was thus able to continue my responsibilities at the centre for the remainder of the night.   3. Expectations for the provision of medicines Public/political pressure will always be concerned with health costs. Every wish to introduce innovations with care, every innovation towards quality and excellence will be done by evaluating their costs. The promise of molecular science is for the time being overestimated, but we must prepare ourselves for the implications of this revolution. Expectations for the provision of medicines One must develop the quality and care of pharmacy, but what is the significance of these words if the evolution is not integrated in a move towards Managed Care? The demands for continued reform of professional education are accepted but in which direction?   5. Expectations for the provision of medicines Patients and consumers are demanding as a right, an internationally high level of safe access to medicines using the logistics of health care chains. That means complex stakes concerning medications and their quality, the creation of new work processes and the introduction and integration of important reforms into health systems.   6. Expectations for the provision of medicines The remuneration of the pharmacist must change; this is on the agenda of every professional association. It is moved by an enormous challenge linked to the professional role of the pharmacist in future health care systems. Expectations for the provision of medicines • The integration of the whole concept of patient safety into the supply chain • The constant reform and development of the way pharmacists and pharmaceutical scientists are educated • The introduction of services and new professional attitudes centred on the needs of the patients with the objective to reduce health costs. • Assuring the integrity and quality of medicines so that no counterfeit and/or spurious medicines enter into the supply chain 48th GIRP Annual General Meeting, 18th June 2007, GENEVA Dr. Michel Buchmann, FIP Vice-President
