  1. Equipment Opening Testing Criteria Phillips 66 High Learning Value Event

  2. Testing Criteria • 50% LEL • 50ppm H2S • 500ppm VOC * • < 150 F • < 1psi Pressure • No Residual Liquid; periodic drips only No guarantee of safety. PPE often Needed.

  3. When Is Testing Done? Initial Equipment Opening • All initial openings on process equipment prior to line or flange breaking. • Sub-systems receive independent tests. • Steam & hydrocarbon Re-Testing • Whenever equipment is completely closed it will be re-tested prior to opening. • Blinded vessels may be left open. • Valve isolations are always closed ea. shift.

  4. How is Testing Conducted Operator Testing • 5-Gas Meters • LEL, H2S, VOC • Dilution Fitting 5:1 • Temperature Gun • Try Point Bleeds, Vents * Pressure gauges are an indicator; but not measure.

  5. Craftsperson Verification Try Point Tag • Minimally, every shift each Lead Craftsperson* is required to verify absence of energy and Equipment Opening Testing Criteria prior to working on a piece of equipment. * Must be on the tools. • Craft verifies & signs tag during Initial Opening • Individual workers may always personally verify energy control.

  6. Criteria Not Met/Unable to Test? Criteria Not Met Unable to Test No Try Point, Bleed or location to test. Option 1 – Redesign Equipment Isolation Option 2 – Job Safety Plan JSP does not guarantee work will proceed. • LEL, H2S, VOC too high? • Temperature too high? • Residual Liquid/Pressure? Option 1 – RE-Clean Equipment Option 2 – Alternate Plan Option 3 – Job Safety Plan* * OPS/MAINT/H&S Supv’s

  7. Conclusion • All equipment that contains process fluids must be tested for hazards prior to initial line breaking: • LEL, H2S, VOCs, Temperature, Pressure & Residual Liquid • Readings are recorded on the Try Point Tag which is attached to the bleed on the equipment. • Crafts persons must verify acceptable readings prior to performing any initial line breaking activities. • Further equipment cleaning or redesigning the task is the preferred solution to non-passing criteria. • Job Safety Plan is required for any deviations to criteria.
