This baseline outcome questionnaire is designed for health and social care research and will be available in postal and web-based formats. It consists of nine sections, each containing standardized measures widely used in research. Key sections include subjective well-being, GHQ-12, general health, social care outcomes, and dependency levels impacting outcomes. The questionnaire also examines the use of health and social care services and equity implications concerning personal health budgets (PHBs). Data protection measures ensure personal information remains secure and confidential.

Data Protection • Names and addresses are kept separately from the personal information collected on the questionnaire • A unique ID will be used on the questionnaire • For postal questionnaires, a pre-paid envelope will be sent
