  1. Don’t blow your TOP! Which way round? 1. What A recent incident has highlighted a hazard associated with the use of a cryogenically cooled module. When brought up to room temperature an incorrectly installed and poorly designed Pressure Relief Value (PRV) did not operate. As a result the system pressurised and the valve was ‘blown off’ while being removed. 3. Learning 2. Why • PRVs should be fitted where vacuum systems can be pressurised; • PRVs should be registered for statutory inspection; • Pressure gauges should be installed to indicate the state of pressurisable systems; • Staff working on complex cryogenic vessels should understand the system status and it’s hazards. Further detail can beobtained from your local SHE Group Issued November 2012 • The systems’ potential hazards had not been fully assessed; • A poorly designed PRV was used and its orientation was not clearly marked, and installed the wrong way round; and • No single person was in overall charge of the work, leading to misunderstanding between different groups working on the equipment.
