On Boarding with the EASE Plan • Review Services and Costs • Membership Agreement, download and Review • Application, Submitted • Apply and EASE Plan Responds • Assimilate (Electronic) Medical Records • Member (and Family) Interview • Review current care Plan • Assess current Housing, and options to Optimize Long Term • Collect information on existing Medications • Costs and Billing

Step 1: Review EASE Plan Services Obtaining a thorough medical history, list of current medications, diagnoses, and physicians Obtaining information about the home environment in order to make recommendations or prepare for facility placement should a change in health condition occur Gather the information needed to coordinate the Member’s care Consulting with and educating the patient and family on the many aspects of long-term care as the patient’s legal rights

  5. Step 2: EASE Plan Membership Agreement .. the Member and Family must download from the website, or request a printed copy of the Membership Agreement which should be reviewed thoroughly

  6. Step 3: EASE Plan Application .. the EASE Plan Team requires background information to evaluate the eligibility and logistics of Membership. This is collected through the use of an Application Form.

  7. Step 4: EASE Plan responds to the Application, and will Review with the Applicant A careful review of the Application is an important step in the process so the prospective Member has all of their questions answered, and any concerns are addressed.

  8. Step 5: Collect and Assimilate Medical Records The Member’s Electronic Medical Record (EMR) needs to be as complete as possible. This EMR is stored in a secure, and HIPAA compliant system

  9. Step 6: Member and Family Interview The Care Advocate will come to the Member’s home to conduct an Interview, and will ask to include those in the Member’s Family who are participating in the Member’s care and long term Planning

  10. Step 7: Review Current Care Plan Collect the Physician’s contact information Assess current Housing, and options to Optimize Long Term Collect information on existing Medications

  11. Step : 8 Costs and Billing The Costs related to the various Membership Levels will be explained, and understood by all those participating in the delivery of care, including the Member and their family And the Billing arrangement should be carefully discussed to suit the Member’s needs
