  2. The Challenge To develop a generic worker operating at the interface of health and social care ;utilising social care, therapy and clinical skills.

  3. 80 employees undertaken Foundation Degree in Health and Social Care • Two year programme one day per week • Generic first year, specialist second year modules • Work Based Education Facilitator providing link to employer

  4. Definition of Reablement • According to CSED 2007:- • “ Services for people with poor physical or mental health to help them accommodate their illness by learning or relearning the skills necessary for daily living.” • Reduces need for ongoing commissioned service. • Up to 50% of users no requirement for ongoing support.

  5. The Teams • A short term service • AP In reach team will take referrals and carry out first visit establishing the initial Personal Support Plan • AP will constantly monitor and review service user progress • AP will carry out reviews with the service users and bring their findings to the review meeting • AP In reach team will fast track referral for equipment and may procure certain pieces of equipment directly • AP will contribute the individuals PSP as an on-going process

  6. What does the model look like? • Initial Assessment carried out by commissioning teams and outcomes identified • Home Support provided on a day to day basis • Weekly goal setting with service user to achieve their stated outcomes; carried out in conjunction with therapists • Week 3 or 4, if the service user is still receiving reablement service, will consider future needs of service user with social work input. Consideration to be given to Direct Payments and Assistive Technology • Final review will produce an outcome focussed Support Plan with recommendations for on-going support • Exit strategies with AP In reach linking with voluntary and statutory services for a comprehensive discharge.

Outcomes of reablement • Ensuring service users are in control • Promoting independence, wellbeing and choice in whatever we do • A service that suits the individual and is adaptive and flexible • A return to full independence or with a service that respects autonomy and choice and is tailored specifically around the individual needs and expressed outcomes

  10. Challenges • Changing mind-set and culture • Matching workforce capacity and demand • Selective or non-selective • Ensuring through put • Agreeing performance indicators and data collection
