Linking LGBT Young People to HIV Primary CareHeights Health Center • System Gaps: • No current system in place to keep this population connected to ongoing PC • Our Aim: • Improve linkage to HIV primary care for newly diagnosed homeless LGBT youth to 95% by 12/2012. • Interventions: • Partner with Community House to provide peer escort to escort newly diagnosed patients to HIV primary care • Establish offsite support/educational group for newly diagnosed LGBT in collaboration with Community House • Lessons Learned: • Testing small scale changes across agencies requires a clear plan & delineated responsibilities Barriers to Linkage: • Discrimination and Alienation; Stigma; lack of trust/comfort in “traditional” services for homelessness/adults. • Next Steps: • Develop a clear plan with Community House and peers to test and measure the impact of escorts
