Do You Know? • The location of the fire alarm pull station nearest to your lab • Fire Extinguisher location • Two ways to exit from the building where the nearest Automated external Defibrillator (AED) is located • The gathering point outside the building for your lab to determine who is missing • Location of Nearest Eyewash • Location of Nearest Safety Shower

Eyewash Requirements Weekly Eyewash Flushing Instructions • Activate eyewash station by pushing the handle assembly or stepping on the foot assembly to start the water flow • Allow water to flow for 2-3 minutes to flush stagnant water & sediment from supply line • Secure water flow by placing handle assembly in the off position • Replace dust covers on the eyewash yoke assembly • Initial and date the inspection log when complete

Do you know where the NWI hazard communication plan is located? • Safety Data Sheets (SDS): Formerly MSDS. New 16 Section OSHA format • Chemical Labeling and Storage: *New OSHA GHS Requirements • Training: Required annually. Do you know the chemical hazards in your work area? • Emergency Response: What do you do if there is a chemical spill?

Safety Data SheetsFormerly MSDS • Do you know where the SDS in your area are? • SDS are required for any chemical with a physical or health hazard. • Should be specific and up to date for each chemical (and manufacturer) in your inventory • Provides comprehensive information about the chemicals used in your work area including: • Physical Hazards (flammable, reactive, corrosive) • Health Hazards (carcinogenic, irritant, sensitizers) • First Aid Measures • Fire Fighting Measures • Handling and Storage • Exposure Controls/PPE • Toxicological Information • Disposal Considerations

Red Emergency Response Procedures Manual • Do you know where the Red Quick is in your area? • The Red Quick Reference Manual is located on each floor in Building 15 near the Shred Bins and in the lobby of the 12th floor RAO. • These provide comprehensive information about what to do in an emergency for various situations: • Tornado plan • Utility Failure • Hazardous Materials incident • Fire plan • Winter Storm Information • Mass Casualty response • Bioterrorist event plan • Pandemic Flu plan • Bomb Threat checklist

  11. What is Personal Protective Equipment • Lab coats-only in the lab • Gloves-only in the lab • Goggles • Bonnets • Powered Air Purifying Respirator

Animal Research FacilityPersonal Protective Equipment All persons entering the ARF animal room(s) will be required to wear a lab coat as a covering of street clothing.   When handling animals hair covering, face mask, exam gloves and shoe covers are required.  This covering is to be worn only when working in the ARF and exchanged for a different lab coat when working in the laboratory.

  13. Who do you call ? Small spill- If there is NO hazard to employees or the environment then the Lab cleans up using the appropriate spill kit, while wearing personal protective equipment and disposes of all material properly. Large Spill – If there is hazard to people or the environment or the effect is unknown. Isolate the spill area (evacuate). Deny entry to others. Call x3333 with the location and type of hazardous release. VA Police will secure the area. The VA Police dispatch will contact an environmental cleanup company. Provide responders a hard copy of the MSDS. Contact the NWI Safety Office at x4442 After Hours: Administrator of the Day x3250; Police x3202 or x3333

Accident Reporting • Seek care, then contact your supervisor. • Occupational Health-Mod A (Building closest to Woolworth) room 105 – Lois Johnson x5823 Office Hours 7:30am-4:00pm • Emergency room Monday-Friday before 7:30am, after 4pm & weekends x3239 • For VA leased space - Know & follow local procedures • The AO will enter your information into ASISTS and the ACOS/Research will sign off.

Fire Safety Dial x3333 to report a fire • Never prop a door open. • Keep corridors clear and passable. • Know where the smoke doors (or other barrier doors) are located in your work area. • Report unfamiliar odors (gas, fuel, burning). • Keep an 18-inch clearance below fire sprinklers. • Know the location of all pull stations (fire alarm boxes) and extinguishers in your work area. • Know your facility’s emergency number to call a fire x3333. • Know your role in responding to a fire in your facility. • Participate in your facility’s fire drills.

  17. Where Do You Go During a Tornado Clinical Research Unit – Move to basement if possible. If there is insufficient time or mobility constraints, staff and patients move to interior hallway away from windows and close all doors. All employees are responsible for assisting with the movement of patients, families and visitors. Building 15 –Move to the Basement of Building 15 12th Floor – Move to the basement whenever possible. If there is insufficient time or mobility constraints, move to 9th floor interior corridor. Close all doors. Building 1 Basement -Stay where you are.
