6. 3 month evaluation • Direct warden referral n=3 • All appropriate • All required case management approach Outcomes: • 1 unscheduled hospital admission prevented • No further fall events • Rate of referral able to be absorbed within existing ICT capacity • Well being – resident & warden

7. Current evaluation & findings • 19% of referrals to ICT need fall assessment & intervention • Referrals from SH:- Number & appropriateness • The type & nature of care required for ‘faller’ (mean ‘care package’) • MFRA • Case management • Physiotherapy programme • Cost of care package – cost of preventative approach

8. Where we are now - June? • SH project to timescale • Cascade training & falls tool box • Up-grade of falls register • Dissemination • Achieved IRF funding for Partnership Project • Support for cost analysis • Capacity to open referral pathway from Alert to ICT Risks • Sustainability – finance, contracts, leadership
