What should be included? • What happened? • Where did the incident happen? • How did it occur? • Did any injuries occur? ...to whom? • Did any property damage occur? ...to what? • When did the incident occur? Was it at a Fraternity event? Did it happen before or after a Fraternity event? • Were there any witnesses? Who? • Did any events lead up to the incident? If so, what were they? How might the events have contributed? • Suggestions to prevent recurrence. • Corrective action taken.
