Like all our services provided, our clients are the direct point of care, what this means is that we customise our services to allow you the flexibility of choosing how, where, when and who provide your support. We work collaboratively with You and your care team such as family, doctors, and hospital discharge team to support and strengthen your welling-being.

2. Personal Care At DH Holistic Care, our personal care includes assistance with your appearance, hygiene, dressing, and self-care. Whether you live alone, with families, or in a shared living situation, we have a team ready to deliver personal care services in different care settings. This service is for anyone wanting to break the social barriers and build social interactions with people within their community. Whether you are looking to attend appointments or errands, participate in bowling activities, etc. Our support workers are there as a companion to ensure you are safe. • DH Holistic Care provides Community and Social Support services include: • Assistance with going to your local Cafe to indulge in your favourite coffee • Enjoy a nice day on a road trip with our support team • Attend workshops such as art & crafts, or take a class on something • Attend driving Licence • Support families to enjoy a family day out for picnic

4. Transport Services We offer our clients flexible travel and transport assistance to hospitals, doctors’ appointments, social visits, social, community, employment, and shopping using either the client’s vehicle or our carer’s vehicle. Our Assist-travel services include special transport arrangements, Uber or taxis or clients’ private cars, or staff safe and secured vehicles. We also assist participants with public transport, such as teaching clients how to catch trains or buses to different locations. DH Holistic Care  Assist-Travel is for anyone who is unable to travel alone or cannot access other forms of transport due to personal, disability, or medical reasons.
