  26. Replace “Step Down” sign on inside of door.

  27. Items blocking access to fire extinguisher and cut off switch. Site informed safety assessment team that breaker box is disconnected and needs to be removed.

  29. Contractor frayed sling located on back of crane

  30. Recommend replacing faded signs around the plant.

  31. Ladder stored under CT?

  32. Missing warehouse building on site map. add

  33. Recommend going through revised load rating process on lifting equipment when using man lift. Safety tactical team is currently working on procedure.

  34. Need “Confined Space” signs.

  35. Recommend IH sampling for soldering activities.

  36. Unchained gas bottles in storage rack. Unlocked battery room at external access.

  37. No GFCI evident near sink in lunchroom
