A Preconstruction Meeting will be held to discuss contracting requirements with firms placed on the QVL.

18. 18 Consultant Services Selection Process The Aviation Department in collaboration with the Prime Leadership Firm will make qualifications based selections from the QVL on an as-needed basis.
Firms from the QVL will be selected based on meeting the services criteria specified in the scope of work. Selected firms will contract directly with the Prime Leadership Firm to provide services.
Service Areas: Project Management, Quality Control, Construction Services, Program Controls, Support Services

Services Proposal (SOQ’s) will be evaluated by an Evaluation Panel.
Shortlisted firms will be notified and interviews scheduled:
Services Requirements – Firm representatives
Staffing Requirements – Firm prime candidate
Interviews conducted by an Evaluation and Scoring Panel.
Highest scoring firms notified of selection.

Experience and Qualifications of the proposed individual(s) in the service areas identified. (40%)

SOQ’s due September 4, 2009 @ noon Local Time
SOQ’s reviewed September/October 2009
Qualified firms will be placed on QVL for a period of two years
Notification of firms placed on QVL will be posted on the Phoenix Sky Harbor International Airport Design and Construction Business Opportunities website: http://www.phxairportbiz.com

Exhibit 1 – General Checklist and Service Areas
25. 25 SOQ Format and Requirements One page maximum to describe Firm’s Qualifications for each Service Area,
One page maximum to describe Qualifications for all Proposed Individuals for each Service Area,
Additional two page maximum to describe Specialties selected for Project Management and Quality Control Services Areas,
Resumes for proposed key individuals shall be limited to one page and should be attached as an appendix to the firm’s SOQ,
Certificates should be included, as appropriate, in the appendix with the resumes they pertain to,
Provide one hard copy of the Statement of Qualifications and one electronic copy on a compact disk (CD) in either Adobe Acrobat (.pdf) or MS Word (.doc) by 12:00 noon local time on Friday, September 4, 2009.
26. 26 SOQ Format and Requirements - Continued Adherence to maximum page criterion: Each page side with criteria will be counted.
Pages that have photos, charts, and graphs will be counted toward maximum number of pages.
Table of Contents and plain tab dividers will not be counted.
Front and back covers that have pictures without descriptive captions will not be counted.
Resumes should provide information for key staff – no company profiles.
Font size may not be less than 10 point.

28. 28 Affirmative Action Requirements Affirmative Action: To do business with the City of Phoenix firms MUST comply with City of Phoenix Affirmative Action Ordinance.
The firm’s Affirmative Action expiration date MUST be on each submittal cover.
