  4. Products and Services Inventory (PSI) • What is it? • Survey that captures all products and services purchased, developed, or utilized by your suicide prevention program • Importance? • Keep track of what is being produced from grant funding

  5. Products and Services Inventory (PSI) • How/When is it administered? • Seed Grantees: • Online survey: http://youthsuicideprevention.nebraska.edu/ • Due Quarterly – June 30, Sept 30, Dec 30, March 30 • Trainers: • None – PPC will get training info from TES

  6. Products and Services Inventory (PSI) • ID – fill out coalition name • Part 1 – fill out separate survey for each product and inventory • Part 2 – Fill out one survey for your project

Products and Services Inventory (PSI) • Part 1: Enter all products, services, and activities implemented in suicide prevention program • Outreach and Awareness (e.g., website with suicide prevention information; printed materials; media) • Training (e.g., QPR; screening; assessment) • Policy and Protocol Development (e.g., screening protocols; research support materials) • Direct Services (e.g., screening; assessments; mental health services; postvention services) • Other Prevention Infrastructure (e.g., building a suicide prevention coalition)

  9. Products and Services Inventory (PSI) • Part 2: Dollar amount of GLS funds spent and the % of funds expended on each of the 5 categories from Part 1.

  10. Pre/Post Training Survey • What is it? • Pre and post survey created to assess knowledge gained from QPR training. • Importance? • Measure increase in knowledge about suicide prevention as an outcome of training.

Pre/Post Training Survey • How/When is it administered? • Trainers: • Administer paper pre-survey at the beginning of QPR training • Administer paper post-survey at the end of QPR training • Mail pre/post surveys into PPC along with TES (see last slide for contact info) • Continuously, but deadline is Quarterly – June 30, Sept 30, Dec 30, March 30

  12. Training Exit Survey (TES) • What is it? • Instrument created to assess the content of the training, the participants’ intended use of the skills and knowledge learned, and satisfaction with their training experience • Importance? • Keep track of training activities

Training Exit Survey (TES) • How/When is it administered? • Trainers: • Administer paper survey to all training participants at the end of training • Reminder: make sure they use a #2 pencil (no pens) and fill in bubbles completely otherwise they can’t be scanned • Fill out TES cover page for each training • Mail TES surveys and cover pages to PPC (see last slide for contact info) • Continuously, but deadline is Quarterly – June 30, Sept 30, Dec 30, March 30

  14. Early Identification and Referral Form (EIRF) • What is it? • Form used to track early identification activities, referrals for service, and received services • Importance? • Ability to connect youth at risk for suicide to appropriate services and follow-up on services received

Early Identification and Referral Form (EIRF) - Aggregate • How/When is it administered? • Seed Grantees: • If doing a formal screening process like Teen Screen then you will need to keep track of: • Unduplicated count of # of youth who took screening questionnaire • Unduplicated count of # of youth who screened positive on questionnaire only • Unduplicated count of # of youth who screened positive on questionnaire OR one-on-one interview OR self identify as suicidal at any point • Gender and race for all youth screened • Report to PPC Quarterly – June 30, Sept 30, Dec 30, March 30

  16. Early Identification and Referral Form (EIRF) - Individual • How/When is it administered? • Trainers: • Instruct gatekeepers to fill out online survey: http://youthsuicideprevention.nebraska.edu/ • Every time a referral is made a new EIRF should be filled out • Referral follow-up can be reported with referral or up to 3 months later (email reminders will be sent) • Complete Continuously, but deadline is Quarterly – June 30, Sept 30, Dec 30, March 30

  17. EIRF- Individual Referral Information • Participant ID • Setting – mental health agency • Source – screening • Age/Gender/Ethnicity/Race • Screening Results • MH Referral/Referral date/Type of agency • Non-mental health referral • Reason for non-referral
